KVKK APPLICATION AND INFORMATION REQUEST FORM

 

GENERAL INFORMATION

Under the Law No. 6698 on the Protection of Personal Data (“KVKK”), personal data owners who are defined as the “data subject” (hereinafter referred to as the “Applicant”) are granted the right to submit certain requests regarding the processing of their personal data pursuant to Article 11 of the KVKK.

Pursuant to Article 13/1 of the KVKK, applications to our Company, as the data controller, regarding these rights must be submitted in writing or by other methods determined by the Personal Data Protection Board (“Board”).

It is important that the personal data we keep about you are accurate and up to date. Therefore, please inform us if there is any change in your personal data. In accordance with the KVKK and relevant legislation, you may:

– Learn whether your personal data are processed,

– Request details regarding the information we collect,

– Learn for what purpose such information is collected and how it is used, and the third parties to whom personal data are transferred domestically or abroad; or

– If your personal data are processed incompletely or incorrectly, request that they be corrected/updated; or if the reasons requiring processing no longer exist, request deletion, including notifications to third parties to whom the data have been transferred.

Within this framework, written applications to our Company may be submitted by printing this form and writing “Information Request under the Law on the Protection of Personal Data” on the envelope or in the subject line, and delivering it to us via:

• The Applicant’s personal application,
• A notary public or registered (return-receipt) mail,
• Sending to our Company’s registered electronic mail (KEP) address.

This Form has been prepared to enable the fulfillment of your request to be submitted regarding your personal data before ADADÜNYA OPTİK TURİZM TİCARET LIMITED COMPANY (Data Controller) pursuant to your rights as a Data Subject under the KVKK.

Your application will be answered as soon as possible and at the latest within 30 days in accordance with the KVKK. Please note that if the information and documents you provide are incomplete or unclear, we may contact you, as the Data Controller, to finalize your application.
