REGARDING THE PROCESSING OF PERSONAL DATA

Pursuant to the Law No. 6698 on the Protection of Personal Data (“KVKK”) and the Regulation on the Processing of Personal Health Data and Ensuring Their Privacy, I have been provided training within the Company regarding the relevant legal regulations and my rights within this framework, and I have read and reviewed the information notice delivered to me after the training.

Within the scope of the relevant legislation, I hereby give my explicit consent for the processing by ADADÜNYA OPTİK TURİZM TİCARET LİMİTED ŞİRKETİ of my personal data, including any data that identifies or may identify me, as well as my special categories of personal data and my health data, which I provide verbally/in writing and/or in electronic media to ADADÜNYA OPTİK TURİZM TİCARET LİMİTED ŞİRKETİ.

I also give my explicit consent for such data to be shared, as required for the performance of the work, the provision of the service and the planning of such services, with other persons and organizations where sharing is necessary under applicable legislation; and for my personal data, special categories of personal data and health data to be processed within the scope defined under the KVKK and the Regulation on the Processing of Personal Health Data and Ensuring Their Privacy, as detailed below.

Identity data: Data containing information about a person’s identity, which clearly belongs to an identified or identifiable natural person and is processed wholly or partially by automated means or, provided that it forms part of a data filing system, by non-automated means.

Such data may include: name-surname, Turkish Identification Number, nationality information, mother’s and father’s name, place and date of birth, gender; documents such as driver’s license, national identity card and passport; criminal record certificate; diploma copy; where necessary, disability certificate; military service status certificate; health report; blood group card; biometric data; if married, marriage certificate; tax number; Social Security Institution (SGK) number; credit card information; bank account information; signature information; vehicle license plate; spouse’s and children’s name, surname and Turkish Identification Number; and in emergencies, the name, surname and phone number of the person to be contacted, etc.

Contact data: Data containing communication information, which clearly belongs to an identified or identifiable natural person and is processed wholly or partially by automated means or, provided that it forms part of a data filing system, by non-automated means; such as phone number, address, e-mail address, fax number, IP address, etc.
Marketing data: Shopping history information that includes marketing data, survey data, cookie records, information obtained through campaign activities, etc., which clearly belongs to an identified or identifiable natural person and is processed wholly or partially by automated means or, provided that it forms part of a data filing system, by non-automated means.
Records kept for security and workflow purposes: Data obtained by keeping records for security and business process purposes, which clearly belongs to an identified or identifiable natural person and is processed wholly or partially by automated means or, provided that it forms part of a data filing system, by non-automated means.

Such data may include: card or non-card access system records collected at the entrance to and during presence within physical premises; personal data relating to documents; camera recordings and records taken at security points; in the case of using a company vehicle, vehicle tracking system records; access logs generated based on the use of company computers.

Data relating to the receipt and evaluation of any requests or complaints directed to ADADÜNYA OPTİK TURİZM TİCARET LİMİTED ŞİRKETİ, which clearly belongs to an identified or identifiable natural person and is processed wholly or partially by automated means or, provided that it forms part of a data filing system, by non-automated means.

I have read the Information Notice and my rights regarding the Protection and Processing of Personal Data, which is available on the website and/or provided in printed & electronic media, or sent via SMS (short message) to the GSM number I have declared as my contact number, and within this scope, I hereby ACCEPT with my free will that my personal data may be recorded, stored, retained, re-organized, shared with institutions authorized by law to request such personal data, and, under the conditions set forth in the KVKK, transferred or disclosed to third parties domestically or abroad, assigned, classified, and processed in other manners listed in the KVKK.
